They need to eliminate the QB Slide. The QB Slide is supposed to protect QBs, but it causes more and more dangerous injuries than just diving forward or getting tackled. QBs are always going to wait too long to slide, and when they slide they drop their head right into the danger zone of a tackler, where the head is the only part of your body in the target zone of a tackler. It's a recipe for concussions.

You combine that with the fake slide that QBs do and you leave the Defender with no options. If they give up and don’t hit you, you fake and run around them for a big gain. If they don’t give up, you get a concussion.

I don't know if they need to eliminate it, but QBs need to quit pushing the envelope with it.  Like you said, we see plenty of QBs try to get every last inch before they slide, sometimes they start sliding right at a defender's feet.  If the defender makes forcible contact in those situations, it shouldn't be a penalty.  QBs should slide in the open field and go down early to let defenders know they are giving themselves up.  Otherwise if you want extra yards, dive head first, keep running, get tackled, etc.
